Students must be fully vaccinated with both doses of the COVID-19 vaccine. Those who are not vaccinated may be denied access to clinical facilities, which could jeopardize the successful completion of their course or practicum.

Keep the vaccine receipt for clearance review, if available.

COVID-19 Vaccine:
Most placements no longer require a COVID-19 vaccine.
Please be aware that still some placement agencies may still require COVID-19 vaccine.

Answer: Vaccine receipts for all doses or the Ontario Covid-19 vaccine receipt with QR code.
What should I do if I have received the COVID-19 vaccine?
Answer: If you have received the COVID-19 vaccine and have documentation, please submit it for clearance review.
What if I have not received the COVID-19 vaccine or chose not to?
Answer: If you have not been vaccinated or have chosen not to receive the vaccine, please email us to indicate your personal, medical, or religious reason.
